   Access the Academic Records Management System to apply and print the application form.
   (System opens on September 10, 2025)
2. Prepare the required application documents.
3. Make an early appointment with your Academic Advising.
4. Attend a meeting with the Academic Advising.

Submit the hard copy of the application form
(Ensure all required fields are completed, Seal in the relevant fields and the applicant’s signature. Submit the form to the Interdisciplinary Program Promotion Office.)

Please combine all application documents into a single PDF file and upload it.

1. Project Proposal
2. Official Transcript (all semesters)
3. Current Semester Course Enrollment Record
4. Any other supporting documents beneficial to the review
